Your First Visit
Once you have an appointment scheduled with one of our board certified physicians, we recommend collecting as many of the following items as possible. This will help the physicians with reviewing your concerns and providing treatment as promptly as possible.
To make your first visit easier, we ask that you bring the following:
- Any X-Rays, CT's or MRI's your physician may have ordered for you (this includes the CD disk as well as the radiology report)
- Your insurance card
- Driver's license
- A referral from your Primary Care Physician (if required by your insurance company)
- Completed new patient forms/health history. Create an online portal account to update your information/medical history prior to your visit or visit the New Patient Form section to download paper forms. Print them and bring the forms with you to your visit.
Feel free to bring family members to your visit and ask our staff any questions you may have. We're also available to answer any questions you might think of after your appointment as well. We're here to serve you and want you to be comfortable with your treatment plan.
