Your First Appointment

Our staff will greet you and help you in any way during your first visit.
To help us, we ask that you bring the following items to your first appointment:

Any X-Rays, CT's or MRI's that your referring physician may have ordered for you.
A copy of your insurance card and driver's license
A referral from your Primary Care Physician if required by your insurance carrier
Completed New Patient Forms which can be downloaded here.

If you have any billing related questions, our Business Office will be happy to assist you.

Elective Approach to Surgery

Because each patient's condition (physical, emotional and financial) is unique, we help you to make an
"educated" choice to having surgery or to pursue more conservative methods of pain management,
including lifestyle changes, weight loss, oral medications, injections and physical therapy. For the majority
of patients who come to our facility, these non-surgical treatment modalities are sufficient.

Payment

Prior to your visit, we will contact your insurance carrier and inform you of your financial responsibility.
Payment for office visits, supplies, x-rays and MRI's is due at the time of service. For your convenience, we
accept all major credit cards. If you have any questions regarding your insurance coverage, please contact
our Business Office.
